Registrations
We are required to ask the patients the following details to be able to register them.
A proof of identity:
Valid passport or driving licence .
A proof of address:
Bank statements or electricity bill not more than 3 month old .
Patients who take any medication need to bring a copy of their current list of medication .
English , Swedish, Portuguese, Polish, Spanish Italian, Persian, and Arabic translation available.
